Requiring family authorization for apnea testing subtracts health professionals control over death determination, a procedure that has traditionally been considered a matter of clinical expertise alone. In this commentary, we first provide evidence showing that health professionals’ (HPs) disposition to act on death determination without family’s prior consent could be much lower than that referred to by Berkowitz and Garrett (2020). We hypothesize that HPs may have reservations about their own expertise as regards death, and may thus hesitate to impose (...) their views on patients’ families. We then address the theoretical question of clinical expertise in death determination by distinguishing judgments about facts (e.g., the presence or absence of spontaneous breathing) from interpretations given of these facts (i.e., their meaning for the vital status of an individual). We argue that, while clinicians may claim some expert authority on the former, they hold no particular authority on the latter. (shrink)

Profundizar en la dimensión espiritual de la persona humana resulta atractivo puesto que ofrece respuestas al sentido de la vida. Se ha constatado que la atención espiritual aporta grandes beneficios para el paciente. Es por eso que en este trabajo hemos querido profundizar en la importancia de la dimensión espiritual en los cuidados paliativos; (...) se destaca la actitud de la escucha como pieza clave de dicha atención espiritual, lo que evidencia que el burnout de los médicos es el principal obstáculo para potenciar la escucha como una adecuada disposición que resulte consoladora y sanadora. The debate over the determination of death has been raging for more than fifty years. Since then, objections against the diagnosis of brain death from family members of those diagnosed as dead-have been increasing and are causing some countries to take novel steps to accommodate people’s beliefs and preferences in the determination of death. This, coupled with criticism by some academics of the brain death criterion, raises some questions about the issues surrounding the determination of death. In this paper, we (...) discuss some of the main approaches to death determination that have been theoretically proposed or currently put into practice and propose a new approach to death determination called "weak pluralism" as a reasonable ethical and political alternative to respect diversity in death determination. Altruism and solidarity are concepts that are closely related to organ donation for transplantation. On the one hand, they are typically used for encouraging people to donate. On the other hand, they also underpin the regulations in force in each country to different extents. They are often used indistinctly and equivocally, despite the different ethical implications of each concept. This paper aims to clarify to what extent we can speak of altruism and solidarity in the predominant models of organ donation. (...) It also raises the ethical question of whether these categories are adequate as a basis for such models, bearing in mind that organs are a scarce resource and that a shortage of them may mean that fewer lives are saved or improved. A cognitive assessment strategy that is not limited to examining a set of summary test scores may be more helpful for early detection of emergent illness such as Alzheimer’s disease (AD) and may permit a better understanding of cognitive functions and dysfunctions in those with AD and other dementia disorders. A revisit of the work already undertaken by Kaplan and colleagues using the Boston Process-Approach provides a solid basis for identifying new opportunities to capture data on neurocognitive processes, test-taking strategies (...) and response styles. Thus, this critical review will combine traditional process-based assessment strategies with support provided or offered by newer technologies that have the potential to add value to data collection and interpretation. There is now considerable interest in neuropsychological test administration using computer/digital technology, both in research and in clinical settings. To add value, any computerized version of an existing cognitive test should respect the administration procedure for which normative data were obtained, should be time-saving in terms of scoring and interpretation, and should, we argue, facilitate gathering information about the processes and strategies followed in test completion. This article will offer an overview of the steps needed when implementing computerization of neuropsychological tests using a Process-Based Approach (PBA) to these technology-based adaptations and will discuss further developments in this area by linking it to future technological developments that may be possible in the area of neuropsychological assessment. Additionally, an overview of neuropsychological tests that may benefit from computerization will be presented, together with suggestions on the specific processes, strategies and features that may be captured with the aid of such computerization. Finally, hypotheses on how virtual reality could be an asset for the future of the PBA to neuropsychological assessment will also be discussed. (shrink)

A proliferation of tests exists for the assessment of auditory-verbal memory processes. However, from a clinical practice perspective, the situation is less clear when it comes to the ready availability of reliable and valid tests for the evaluation of visual/visuo-spatial memory processes. While, at face value, there appear to be a wide range of available tests of visual/visuo-spatial memory, utilizing different types of materials and assessment strategies, a number of criticisms have been, and arguably should be, leveled at the majority (...) of these tests. The criticisms that have been directed toward what are typically considered to be visual/visuo-spatial memory tests, such as the potential for verbal mediation, over-abstraction of stimuli, the requirement of a drawing response, and the lack of sensitivity to unilateral brain lesions, mean that, in reality, the number of readily available valid tests of visual/visuo-spatial memory is, at best, limited. This article offers a critical, historical review on the existing measures and resources for the neuropsychological assessment of visual/visuo-spatial memory, and it showcases some examples of newer tests that have aimed to overcome the challenges of assessing these important aspects of memory. The article also identifies new trends and examples of how technological advances such as virtual reality may add value to overcome previous obstacles to assessment, thereby offering professionals more reliable, accurate means to evaluate visual/visuo-spatial memory in clinical practice. La filosofía de la tecnología es una rama reciente de la filosofía en general. Puede aportar herramientas para problemas derivados de aplicaciones tecnológicas recientes, como la telemedicina. Un problema señalado reiteradamente por la bioética lo constituye la privacidad en general, y las modificaciones que pueden introducirse por el cada vez mayor uso de las tecnologías. Comentando propuestas desde la Asociación Médica Mundial y datos contemporáneo derivados de la pandemia de COVID-19, puede reconocerse que la privacidad es una exigencia ética que (...) requiere regularse a través de su implementación política para su adecuada protección legal. Background: This research explores how public awareness and attitudes toward donation and transplantation policies may contribute to Spain's success in cadaveric organ donation. Materials and Methods: A representative sample of 813 people residing in Andalusia (Southern Spain) were surveyed by telephone or via Internet between October and December 2018. Results: Most participants trust Spain's donation and transplantation system (93%) and wish to donate their organs after death (76%). Among donors, a majority have expressed their consent (59%), and few nondonors have (...) expressed their refusal (14%). Only a minority are aware of the presumed consent system in force (28%) and feel sufficiently informed regarding the requirements needed to be an organ donor (16%). Participants mainly consider that relatives should represent the deceased's preferences and be consulted when the deceased's wishes are unknown, as is the case in Spain. Conclusion: Public trust in the transplant system may contribute to Spain's high performance in organ donation. High levels of societal support toward organ donation and transplantation do not correspond with similar levels of public awareness of donation and transplantation policies in Spain. (shrink)

La celebración de fiestas en honor a los santos patronos de las comunidades andinas es una de las manifestaciones de religiosidad más extendidas desde Colonia. Másallá de entenderla como una manifestación en continuidad directa con las prácticas cúlticas del Tawantinsuyo o la ritualidad católica española, consideramos que esta festividad es un fenómeno emergente que debe ser analizado en su especificidad. El objetivo de este artículo es analizar la figura del santo y su eficacia simbólico-ritual en la fiesta patronal andina desarrollada (...) durante el periodo colonial (XVI-XVII). A partir del análisis de una serie de crónicas coloniales, intentaremos mostrar que las especificidades rituales de la fiesta y de la operación del santo como símbolo icónico dominante permiten comprender su eficacia simbólica, así como también la co-existencia de diversas interpretaciones y escenificaciones rituales que darían cuenta de la agencialidad ritual de la comunidad. The celebration of festivities in honor of the patron saints of the Andean communities is one of the most widespread manifestations of religiosity from the Colony. Beyond understood as a manifestation in direct continuity with the cultic practices of the Catholic ritual Tawantinsuyo or Spanish, we believe that this festival is an emerging phenomenon that must be analyzed in its specificity. The aim of this paper is to analyze the figure of the saint and their efficacy in symbolic ritual Andean fiesta developed during the colonial period (XVI-XVII). From the analysis of a series of colonial chronicles, try to show that the specific rituals of celebration and operation saint as provide insight dominant iconic symbol symbolic effectiveness, as well as the co-existence of different interpretations and stagings rituals give account of agency community ritual. El artículo plantea condiciones clínico-epidemiológicas peculiares cuando la humanidad se enfrente a una zoonosis emergente. El ejemplo que groseramente ha golpeado a la humanidad es la pandemia de Covid-19, enfermedad causada por el coronavirus SARS-CoV-2. Si bien la historia de la humanidad da cuenta de múltiples pandemias ocasionadas por varios agentes infectocontagiosos, es cierto que en el primer momento de afrontar una problemática global de esta naturaleza lo que prima es la incertidumbre. Ante ella, existe la obligación moral de proveer (...) los mejores servicios de salud que sea posible; a la par, debe realizarse una investigación para encontrar medidas farmacéuticas específicas que contengan y mitiguen el daño poblacional (medicamentos específicos y/o vacunas). Mientras tanto, sabiendo que en el caso de la actual pandemia de Covid-19 la principal y más eficiente vía de transmisión es la propagación por gotas de saliva, el distanciamiento físico es crucial. Una consulta médica involucra hablar en el interrogatorio y una proximidad física durante la exploración. Existen datos que indican que la telemedicina tiene un alto grado de recomendación para la protección de la salud, tanto de profesionales como de pacientes. Se recuerda que una declaración de la Asociación Médica Mundial ha tratado el tema de la telemedicina; con su exposición, se indican alcances, limitaciones y proyecciones en el futuro. (shrink)

El presente artículo apunta a discutir cómo los actos cívicos o los monumentos actuaron en los albores del siglo XX como mecanismos de articulación dinámica de la “chilenización” en el desierto tarapaqueño, particularmente en el puerto de Iquique. Se reflexiona desde una perspectiva sociohistórica en torno a las fiestas públicas como rituales simbólicos donde se materializa la integración y/o pertenencia a la comunidad nacional; eventos cívicos que no dejaron de ser menos persuasivos como otras formas mas “violentas” de implantar y (...) promover el sentido de la Nación en los nuevos territorios anexados al Estado chileno. Este artículo analiza el rol de la escuela chilena en la comunidad andina y las demandas locales por un sistema educativo permanente. Dentro de este contexto, se realiza una discusión en torno al Estado, la construcción de la nación y la utilización de la escuela como instrumento disciplinador de la sociedad. Asimismo, se analiza la experiencia histórica de la escuela en el sur peruano, como parte de un proceso que antecede a la instauración del modelo educativo nacional chileno, relevando la (...) praxis política que las propias comunidades asumen en torno diversos petitorios por la presencia de profesores y de establecimientos educacionales en los sectores cordilleranos andinos. Dos místicos relativamente contemporáneos: Etty Hillesum y Benjamín González Buelta; dos estilos, dos contextos y una coincidencia establecida desde sus experiencias espirituales, en profunda conexión con la realidad, en medio de la vida y sus desafíos. Hillesum no habla propiamente de la experiencia mística, sin embargo, su proceso personal existencial en el duro contexto de guerra que le tocó vivir, la condujo a descubrirse profundamente habitada. Su experiencia de Dios le hizo posible comprender y gestionar el dolor en medio de (...) la guerra, así como la necesidad de ‘hacerse bálsamo sobre las heridas’ de sus hermanos judíos. Etty es una mística de ojos abiertos. Benjamín González Buelta ha profundizado en la comprensión de la mística, describiéndola y analizándola desde la experiencia de Dios en el contexto latinoamericano. Su reflexión permite hoy leer a Etty Hillesum desde la perspectiva del místico de ojos abiertos. (shrink)

BackgroundStudies and meta-analyses found individual, meso and micro-social factors that are associated with individual well-being, as well as a positive socio-emotional climate or collective well-being.AimThis article simultaneously studies and examines these factors of well-being.MethodWell-Being is measured as a dependent variable at the individual and collective level, as well as the predictors, in three cross-sectional and one longitudinal studies. Education and social intervention workers from Chile, Spain and Uruguay participate; a subsample of educators from the south central Chile and from Chile, (...) Uruguay and Spain ; workers from organizations in Latin America and Southern Europe, military cadets from Argentina ; and teams from Spanish companies.ResultsIndividual and collective well-being indicators were related, suggesting that the emotional climate as a context improves personal well-being. Individual factors, psychosocial factors were positively associated with personal well-being in education and social intervention context. Organizational dynamic or transformational culture is directly and indirectly associated with individual well-being through previously described psychosocial factors. Group processes such as internal communication and safe participation, task orientation or climate of excellence as well as leadership style that reinforces participation and belonging, were positively associated with collective well-being in labor and military context and predict team work socio-emotional climate in a longitudinal study- but were unrelated to individual well-being. Transformational leadership plays a mediating role between functional factors and social-emotional climate in work teams. Organizational role autonomy, functional organizational leadership, integration and resources were associated with collective well-being in organizations. Organizational leadership moderates the relationship between task orientation and collective well-being in military context.ConclusionIndividual and microsocial factors influence personal well-being. Meso level factors favorable to well-being through processes which reinforce social belonging, influence directly collective well-being and indirectly personal well-being. Leadership that reinforces participation and belonging play a central role for emotional climate. Stress and emotional climate playing an important pivotal role for psychological well-being. No obstante, un cambio profundo se operó con el Concilio Vaticano II. El pensador latinoamericano contemporáneo Alberto Methol Ferré considera que la Iglesia católica se había colocado, inicialmente, a la defensiva frente a dos grandes desafíos de la Modernidad –la Reforma y la Ilustración–, pero modificó su postura a partir del Vaticano II, asumiendo lo más valioso (...) de ambas tradiciones. En este artículo se analizará la tesis de Methol Ferré para el caso de la Reforma Protestante, en especial en los temas referidos a la Iglesia y el bautismo, el laicado y al sacerdocio común de los fieles. El pensamiento filosófico de Alberto Caturelli supone una extraordinaria revitalización del pensamiento metafísico cristiano en el siglo XXI. El presente estudio tiene por finalidad mostrar de qué forma procede a la restauración de las verdades filosóficas fundamentales -tales como la creatio ex nihilo, la participación, la linealidad trascendentista de la historia, la persona humana y la libertad entre otras- sobre la base de la naturaleza contemplativa de la inteligencia y la preeminencia de la vida interior. Destacaré tres grandes rupturas en su evolución intelectual: en el diagnóstico del presente, en la comprensión del capitalismo como tal y en la propuesta política.In this paper I will try to compare two periods in Nancy Fraser's theoretical evolution: her analysis of contemporary capitalism as an (...) institutional social order and her previous categorial framework, based on the dualims of redistribution and recognition. The articles in the symposium “Teaching Early Modern Philosophy: New Approaches” provide theoretical reflections and practical advice on new ways of teaching undergraduate survey courses in early modern philosophy. This introduction lays out the rationale for the symposium and summarizes the articles that compose it.

A widespread need to explain the behavior and outcomes of AI-based systems has emerged, due to their ubiquitous presence. Thus, providing renewed momentum to the relatively new research area of eXplainable AI (XAI). Nowadays, the importance of XAI lies in the fact that the increasing control transference to this kind of system for decision making -or, at least, its use for assisting executive stakeholders- already affects many sensitive realms (as in Politics, Social Sciences, or Law). The decision-making power handover to (...) opaque AI systems makes mandatory explaining those, primarily in application scenarios where the stakeholders are unaware of both the high technology applied and the basic principles governing the technological solutions. The issue should not be reduced to a merely technical problem; the explainer would be compelled to transmit richer knowledge about the system (including its role within the informational ecosystem where he/she works). To achieve such an aim, the explainer could exploit, if necessary, practices from other scientific and humanistic areas. The first aim of the paper is to emphasize and justify the need for a multidisciplinary approach that is beneficiated from part of the scientific and philosophical corpus on Explaining, underscoring the particular nuances of the issue within the field of Data Science. The second objective is to develop some arguments justifying the authors’ bet by a more relevant role of ideas inspired by, on the one hand, formal techniques from Knowledge Representation and Reasoning, and on the other hand, the modeling of human reasoning when facing the explanation. This way, explaining modeling practices would seek a sound balance between the pure technical justification and the explainer-explainee agreement. (shrink)

This paper focuses on the consolidation of Molecular Evolution, a field originating in the 1960s at the interface of molecular biology, biochemistry, evolutionary biology, biophysics and studies on the origin of life and exobiology. The claim is made that Molecular Evolution became a discipline by integrating different sorts of scientific traditions: experimental, theoretical and comparative. The author critically incorporates Timothy Lenoir’s treatment of disciplines , as well as ideas developed by Stephen Toulmin on the same subject. On their account disciplines (...) are spaces where the social and epistemic dimensions of science are deeply and complexly interwoven. However, a more detailed account of discipline formation and the dynamics of an emerging disciplinary field is lacking in their analysis. The present essay suggests focusing on the role of scientific concepts in the double configuration of disciplines: the social/political and the epistemic order. In the case of Molecular Evolution the concepts of molecular clock and informational molecules played a central role, both in differentiating molecular from classical evolutionists, and in promoting communication between the different sorts of traditions integrated in Molecular Evolution. The paper finishes with a reflection on the historicity of disciplines, and the historicity of our concepts of disciplines. (shrink)

Cultural evolution studies are characterized by the notion that culture evolves accordingly to broadly Darwinian principles. Yet how far the analogy between cultural and genetic evolution should be pushed is open to debate. Here, we examine a recent disagreement that concerns the extent to which cultural transmission should be considered a preservative mechanism allowing selection among different variants, or a transformative process in which individuals recreate variants each time they are transmitted. The latter is associated with the notion of “cultural (...) attraction”. This issue has generated much misunderstanding and confusion. We first clarify the respective positions, noting that there is in fact no substantive incompatibility between cultural attraction and standard cultural evolution approaches, beyond a difference in focus. Whether cultural transmission should be considered a preservative or reconstructive process is ultimately an empirical question, and we examine how both preservative and reconstructive cultural transmission has been studied in recent experimental research in cultural evolution. Finally, we discuss how the relative importance of preservative and reconstructive processes may depend on the granularity of analysis and the domain being studied. (shrink)

The use of molecules and reactions as evidence, markers and/or traits for evolutionary processes has a history more than a century long. Molecules have been used in studies of intra-specific variation and studies of similarity among species that do not necessarily result in the analysis of phylogenetic relations. Promoters of the use of molecular data have sustained the need for quantification as the main argument to make use of them. Moreover, quantification has allowed intensive statistical analysis, as a condition and (...) a product of increasing automation. All of these analyses are subject to the methodological anxiety characteristic of a community in search of objectivity. It is in this context that scientists compared and evaluated protein and nucleic acid sequence data with other types of molecular data – including immunological, electrophoretic and hybridization data. This paper argues that by looking at long-term historical processes, such as the use of molecular evidence in evolutionary biology, we gain valuable insights into the history of science. In that sense, it accompanies a growing concern among historians for big-pictures of science that incorporate the fruitful historical research on local cases of the last decades. (shrink)

This paper extends previous arguments against the assumption that the study of variation at the molecular level was instigated with a view to solving an internal conflict between the balance and classical schools of population genetics. It does so by focusing on the intersection of basic research in protein chemistry and the molecular approach to disease with the enactment of global health campaigns during the Cold War period. The paper connects advances in research on protein structure and function as reflected (...) in Christian Anfinsen’s The molecular basis of evolution, with a political reading of Emilé Zuckerkandl and Linus Pauling’s identification of molecular disease and evolution. Beyond atomic fallout, these advances constituted a rationale for the promotion of genetic surveys of human populations in the Third World, in connection with international health programs. Light is shed not only on the experimental roots of the molecular challenge but on the broader geopolitical context where the rising role of biomedicine and public health had an impact on evolutionary biology. ArgumentThis paper explores the connection between the epistemic and the “political” dimensions of the metaphor of information during the early days of the study of Molecular Evolution. While preserving some of the meanings already documented in the history of molecular biology, the metaphor acquired a new, powerful use as a substitute for “history.” A rhetorical analysis of Emilé Zuckerkandl's paper, “Molecules as Documents of Evolutionary History,” highlights the ways in which epistemic claims on the validity and superiority of molecular evidence (...) for evolution were intimately connected with authority issues in evolutionary biology. The debate is situated within the framework of the battle for resources between traditional evolutionists and molecular biologists at the beginning of the 1960s. The architects of evolutionary synthesis questioned the idea that molecular characters constitute “cleaner” and “more direct” evidence of evolution. Nevertheless, the information discourse constituted a productive space for the development of a new research program that, paradoxically, has made explicit the limitations of the information metaphor in reconstructing life's history.
